End Hep C SF are delighted to announce the fourth partner organization that will be joining our hep C Community Navigator Program in January 2022.
San Francisco Community Health Center (SFCHC) is a 501(c)(3) LGBT and people of color health organization whose mission is to transform lives by advancing health, wellness, and equality. Headquartered in the heavily-impacted Tenderloin Neighborhood since their founding in 1987, SFCHC (formerly Asian and Pacific Islander Wellness Center) was formed as a grassroots response to the convergence of HIV, hepatitis C, and sexually transmitted infections in communities of color, syndemics with biological and behavioral interactions that disproportionately impact the most vulnerable populations in San Francisco.
They have since expanded services to all local communities, with a particular focus on unhoused individuals, transgender people, people who use drugs and/or live with mental illness, and those with chronic illness. In 2015, they became the first new Federally Qualified Health Center in San Francisco in over 30 years, inclusive of primary medical, dental, and behavioral health care.
We are honored to have them join our current partnership with St. James Infirmary, GLIDE, and the San Francisco AIDS Foundation, and to strengthen our work in hep C elimination!
